TMS is FDA approved treatment for specific psychiatric conditions. It uses gentle magnetic stimulation to wake up local and distant parts of the brain.
Medication stimulates the brain by blocking or increasing different neurotransmitters like serotonin, dopamine or norepinephrine. TMS works to increase neurotransmission with magnetic stimulation rather than with medication.
This treatment was first approved by the FDA in 2008 and has continually shown to be safe, tolerable and effective (Kaster & Blumberger, 2024).
TMS is FDA approved for those who have treatment resistant depression, OCD, anxious depression, and smoking cessation.
It is used for off label uses of PTSD, ADHD, Schizophrenia, Addiction, Autism, Bipolar Depression and Chronic Pain. Insurance companies do not cover for off label use of TMS.
TMS is used for those that cannot tolerate medication due to side effects or medications have not worked or have had limited benefit.
TMS uses an magnetic pulses to stimulate the cells in the brain that are thought to be under or overactive, contributing to and/or causing depression, anxiety and or OCD.
You can think of it as waking up a muscle that has been underutilized or learning how to use a muscle in a more helpful way.
It works similar to the way medication works, but without the medication. Medication and magnetic stimulation both work on stimulating neurotransmitters, but TMS does it with magnets.
TMS treatment looks and feels like wearing a helmet that produces magnetic pulses that sounds like a clicking sound. You would wear the helmet and earplugs for 18-22 minutes a day, for 5 days a week, for 4-6 weeks (depending on what protocol you need).
This might seem like a lot, but if you think of it like a muscle, you would not expect to have a strong muscle after 1, 2, or 3 visits to the gym. The brain stimulation is built slowly. People have seen effects as soon as 2 weeks into treatment.

Only recently has insurance companies make TMS more accessible to the public.
Today, most major insurance companies now cover TMS treatment for resistant depression, OCD and anxious depression. In the past a person needed to endure a much higher threshold to become approved. Currently, one can be covered with 2 documented "failed trials" of medication.
Despite its low side effect profile, efficacy and high level outcomes, TMS is not as well known as other treatments. It has been around since the 80's.
Because of the past cost association and time commitment, its not as common as other treatments are.
TMS is a time commitment. Treatments are short (30 min in and out the door), but daily for 4-6 weeks. For this reason, we try to be as efficient as possible, and offer early morning, evening and lunchtime appointments.

It is recommended to NOT make any changes to your medication prior to treatment. There is a possibility you will be able to decrease or stop medication sooner under the supervision of your medical provider.
It is crucial you be completely honest with your provider with what you are taking to decrease the risks of adverse effects.
